2. Pay the $40 application fee
3. Send the application form and supporting documents to:

• Proof of payment of the application fee
• Child’s birth certificate
• Child’s passports (including visa status)
• Child’s Hong Kong ID card
• Parents’ Hong Kong ID card
• Parents’ passport copy
• 2 passport photos of student
• 2 proof of HK address documents
